Step 3: Finish your registration

Steps to register online:

1. Download the registration form. 

2. Fill out the registration form completely. Make sure you have signed it and dated it. Email the form to: [email protected].

3. Email with your registration form a copy of:  (This information can be deposited on the first day of class.)

     a. Your medical diploma or professional license.

     b. A resume or curriculum vitae (CV).

     c. A copy of a photo ID (Driver’s License, Passport, Etc.).

4. Pay the course here. Take advantage of our 10% off saving by paying in full online.

5. If you don’t receive an email after ten business days verify that your form has been received and approved by calling us toll free at 1-855-735-9494.

Steps to register by mail:

1. You can download the registration form or call toll free at 1-855-735-9494 to request one to be sent to you by email or mail. Please leave a message with your name, mailing address and phone number.

2. Fill out the registration form completely. Make sure you have signed it and dated it. Send it by mail to: Disla Clinical Research Corporation, PO BOX 277, Somerset, NJ 08875

3. Mail with your registration form a copy of: (This information can be deposited on the first day of class.)

      a. Your medical diploma or professional license.

      b. A resume or curriculum vitae (CV).

      c. A copy of a photo ID (Driver’s License, Passport, Etc.).

4. Send a check to Disla Clinical Research Corporation. You may pay the course in full or send the $500.00 registration fee and pay your tuition seven days before class start. There is a $35 fee for all returned checks. Please do not send cash by mail. For Financial assistance click here or visit our website for more information.

Steps to register in person:

1. You can register in person by calling toll free at 1-855-735-9494 and requesting a call back to set up an appointment.

2. You will need to bring: (Information a,b and c can be deposited on the first day of class.)

a. Copy of your medical diploma or professional license.

b. Resume or Curriculum Vitae

c. Copy of photo ID (Driver’s License, Passport, Etc.)

d. $500.00 Registration Fee (Cash, Check or Credit Card). You may pay tuition at time of registration or seven days before classes start. There is a $35 fee for all returned checks.
